About the Special Payment for Restaurants in Urayasu

浦安市うらやすしは、COVID-19により 経営けいえいが 難むずかしくなっている 市内しないの 飲食店いんしょくてんを 助たすけるため、
1つの お店みせに 10万円まんえんを 交付こうふします。/
To support restaurants suffering the effect of the spread of the COVID-19, 
\100,000/1 store will be provided from the Urayasu City
※1つのお店みせ 1回かいだけ / 1 time only per store  
　申請しんせいは 3月がつ31日まで / Deadline for this application is March 31, 2021.

もらえる人ひと / Persons eligible for benefits
※下したの 条件じょうけん すべてに 当あてはまる ことが 必要です / 
    All of the following conditions must be met.

①2021年ねん1月がつ1日にちまでに、食品衛生法しょくひんえいせいほうの 「飲食店営業いんしょくてんえいぎょう」の 許可きょかを 受うけている /
   The restaurants that have obtained “Restaurant Business License”
   under the Food Sanitation Act by January 1, 2021.

②2021年ねん１月がつ 1日にちまでに、下したの3つの どれかに 当あてはまり、
　申請日しんせいびに 市内しないで 営業えいぎょうしている 飲食店いんしょくてん /
　The restaurants must meet one of the following conditions and
   be operating in the city by January 1, 2021.

   ・中小企業基本法ちゅうしょうきぎょうきほんほう における 「中小企業者ちゅうしょうきぎょう」 であること /
      Must be a “Small- and Medium-Sized Enterprises” under the
      Small and Medium-sized Enterprise Basic Act article 2. paragraph 1.

   ・個人事業主こじんじぎょうぬしであること /
      Must be a sole proprietor.

   ・社会福祉法人しゃかいふくしほうじん、NPO法人ほうじん、一般社団法人いっぱんしゃだんほうじん、公益法人こうえきほうじんまたは組合くみあいであること
      Must be a social welfare corporation, NPO, general incorporated association,
　　corporation in the public interest , or union.

③新型しんがた インフルエンザ等とう 特別措置法とくべつそちほうに 基もとづく 千葉県知事ちばけんちじからの
   飲食店いんしょくてんへの 要請ようせいに 協力きょうりょくすること / 
   Must cooperate with requests from the Governor of Chiba Prefecture
   to restaurants after the date of application.
   ※要請ようせいの 対象たいしょうと なっていない 飲食店いんしょくてんは 除のぞく /
      Exclude restaurants that are not covered by the request.

④業界ぎょうかいの 団体だんたいが 決きめた 感染防止かんせんぼうしの ルールに合あった 対策たいさくをしていること / 
　Infection prevention measures specified by each industry organization must be implemented.

※下したに 書かかれている お店みせは、お金かねを もらえません / 
   Restaurants that fall under the following categories will not receive benefits.

■持もち帰かえり 飲食いんしょくサービス業ぎょうに 当あてはまる 飲食店いんしょくてん /
  Restaurants that fall under the takeaway food service category.
  例：おかずや 弁当べんとうの 販売はんばいなどの テイクアウトの 専門店せんもんてん や キッチンカー による営業えいぎょうなど
  Example: Stores specializing in takeout, including sales of prepared foods and
                 boxed lunches or food truck service.

■配達飲食はいたついんしょく サービス業ぎょうに 当あてはまる 飲食店いんしょくてん /
  Restaurants that fall under the delivery food service.
  例れい：デリバリーや ケータリングサービス、給食きゅうしょくサービスや 社員食堂しゃいんしょくどうなど
  Example: Food delivery services, catering services, meal services, company cafeterias, etc.

■イートインコーナーを設置せっちしている コンビニや スーパーなど /
   Convenience stores and supermarkets that have an eat-in corner.

■宿泊客しゅくはくきゃくにだけ 飲食いんしょくを 提供ていきょうしている ホテルや 旅館りょかんなど（施設内しせつないの 飲食店いんしょくてんは、除のぞく）/ 
  Hotels and inns that serve food and beverages only to their guests
　(Excluding restaurants in the facility).

必要事項ひつようじこうを 記入きにゅうし、他ほかの書類しょるいと 併あわせて 下したまで 郵送ゆうそうしてください /
You can download the application from the Urayasu City  website.
Fill in the documents and mail it with the attached documents.

申請期間しんせいきかん / Application period
2021年１月がつ15日(金) ～ 2021年3月がつ31日(水) ※当日消印有効とうじつけしんゆうこう /
January 15, 2021(Fri) ~ March 31, 2021 (Wed).
 Note: Application should be postmarked on or before March 31, 2021.

注意すること / Caution

・お金かねをもらった後あと、申請書しんせいしょに 嘘うそがあった<場合rt>ばあいは、お金かねを 返かえしてもらいます。
　If there are any false statements in the application,
　the decision to grant the benefits will be cancelled and the benefits will be refunded.

※不正受給ふせいじゅきゅうは 犯罪はんざいです。警察けいさつと 連携れんけいしながら 厳格げんかくに 対処たいしょします /
    Illegal receipt of the benefits is a crime.
　 We’ll work with the police and deal with it strictly.

・千葉県知事ちばけん/rt>から 飲食店いんしょくてんに 対たいして 出だされる 新あたらしい 要請ようせいについても、
　 協力きょうりょくすることが 必要ひつようです / 
    Must cooperate with new requests from the Governor of Chiba Prefecture to restaurants.
　 協力きょうりょくが 確認かくにんできない場合ばあいは、お金かねを 返かえしてもらいます /
    If we can’t confirm your cooperation, we’ll ask you to return the money.

・浦安市うらやすしは 確認かくにんのため、申請書しんせいしょに 書かかれたことを 調査ちょうさします /
　Urayasu City may conduct a survey to verify the contents of your application.
   浦安市うらやすしの 調査ちょうさに協力<きょうりょく/rt>してください。速はやく 報告ほうこく してください /
   Please cooperate with the survey. Please report back quickly.

   申請書類しんせいしょるいを、ダウンロードできます /
   You can download the application form from the city’s website.

・書類しょるいを 確認かくにんした後あと、審査しんさし、およそ2～3週間しゅうかんで 指定していの 口座こうざへ 振ふり込こみます /
  After reviewing the application documents, the documents will be reviewed.
  The benefits will be transferred to the designated account in approximately two to three weeks.
