For Foreigners in Urayasu City
A baby who was born from 28th April 2020 to
31st December 2020 will receive special payment from Urayasu City.
[Necessary requirements for receiving the payment]
※You need to meet all following requirements to receive the payment
①The baby is citizen of Urayasu at date of application.
②The baby who was born from 28th April 2020 to 31st December 2020.
③The baby who was born from the mother who is registered as a resident
of Urayasu from 27th April 2020 to date of application.

[How to Apply]
・Letter and application form will distributed from Urayasu City to
the mother who meets all requirements at the end of July
or hand the form at City Hall.
・Please fill in the form and send it to Maternal and Child Health Division in
City Hall with *other requirement documents.
・The form will be checked in the City Hall and the payment will be paid in your bank account.

・Copy of identity verification documents
e.g.⇒ Copy of driver license (Upside with picture),
Copy of My-Number Card (Upside with picture),
Copy of Passport (page of identity matters),
Copy of Resident Card
・Copy of bank account documents
e.g.⇒ Copy of passbook,
Copy of cash card,
Copy of screen of internet bank account
*The documents must clarify the bank account information
(name of the bank, account number, name of account holder)
Application Deadline
31st March 2021
・Application should be postmarked on or before 31st March 2021.
・Reception in City Hall will accept the application until 17:00 31st March 2021.
Starting time of transfer the payment
It will takes about one to two month to finish transferring the payment to your bank account.
